"5. When a person wishes to file for bankruptcy, the first step is to compile a list of creditors and work out how much is owing to each of them. Some of these individuals will be secured, i.e. they will hold a mortgage or a lien over some of the debtor’s property. Others will be unsecured. The debtor usually owes money to several creditors. The national bankruptcy law therefore lays down rules to decide the priority of the claimants in a single set of proceedings. If the debtor makes a mistake when filing the first list, this form allows him or her to file an amended list when better information becomes available, e.g. it may appear that one creditor assigned the debt to another, that interest, fees and charges have been omitted from one calculation of the amount owing, etc. Both the original and this amended listing are filed in the form of an affidavit, i.e. the debtor must swear the contents are true to the best of his or her knowledge."
