_________[Name], as owner of premises known as _________, and designated on tax maps of the City of _________ as _________, which is in _________ zone, having heretofore made application to the _________[building inspector] for a special exception to the Zoning Ordinance so that permission may be granted to use the present building as a _________[single-family residence], located at _________[the rear of] premises known as _________, to have a _________ foot setback in lieu of _________ feet from the _________[direction] property line and a _________ foot rear yard in lieu of _________ feet as required by the Zoning Ordinance of the City of _________, and the appeal coming on to be heard, after due and lawful notice to the parties in interest, before the Zoning Board of Appeals on _________, 19__, and the Zoning Board of Appeals having duly considered the matter,

It is, on _________[date],

Decided and Determined

1. That the use of the remainder of property as divided and as to which this appeal is filed is prohibited by the Zoning Ordinance of the City of _________ except by permission of the Zoning Board of Appeals.

2. That the premises which it is proposed to use as stated in appeal filed is located in _________ zone.

3. That the original application to the _________[building inspector] and permit issued called for _________[specify].

4.
